Medicaid for the Aged, Blind or Disabled (MABD)

This information is for individuals applying for Medicaid based on being age 65 or older, blind, or disabled.

To apply for Medicaid if you are not age 65+, blind or disabled, go to the Vermont Health Connect website.


Who is eligible?

To be eligible for MABD Medicaid, you must:

  1. Be a Vermont resident;
  2. Meet one of the following criteria:
    • Aged - 65 years of age or over;
    • Blind; or
    • Disabled (as defined by the Social Security Administration); and
  3. Meet the financial criteria, including income guidelines and resource limits (2020 Protected Income Level (PIL) and Percentage of Federal Poverty Income Guidelines (FPL))

How to find out if you can get Medicaid for the Aged, Blind and Disabled (MABD)

  1. If you have not already applied for Medicaid:

    • See if you can get help paying for health care costs. Click on the 205ALLMED to print out a copy of the 205ALLMED application OR call 1-855-899-9600 and ask us to mail you a copy.

  2. What to do with the 205 ALLMED application:

    • Fill all of it out.
    • Be sure you sign the paper and put the date you signed it.
    • Mail it and copies of any other papers we ask for to:

  3. If you have already applied for Medicaid and got a letter from us about getting Medicaid for the Aged, Blind and Disabled (MABD). See if you meet the rules to get this help. See the instructions on how to send us information for MABD.

You must tell us about any changes

  1. Complete the 200GMC change report form.​​

    • You must report changes to us if you are on Medicaid. Use the 200GMC form below to tell us about changes that you have had. Here are examples of changes we need to know about:

      • Increase or decrease in your income or your resources

      • Increase or decrease in your household size

      • Gain or loss of health insurance

    • Fill out the 200GMC change reort form
      • ​​​Click on the 200GMC to complete the form.
      • You can either fill the form out on your computer OR print a copy and fill it out by hand.
    • If you fill it out on your computer:
      • Save the form to your computer. Remember where you have saved it. You will need to find it later.
        • Then go to http://ahsuploader.vermont.gov on the internet.
        • Then go to choose "healthcare" as the program you are sending in the form for. Follow the steps to send it to us.
    • If you fill it out by hand:

What happens next?

Green Mountain Care will determine your eligibility. If you are found eligible, your eligibility will be reviewed at least once a year.
